How PMALink handles the details you submit through the free evaluation form.
What You Submit
The form asks for your public practice website and a contact email. You may also provide your name, practice name, specialty, service area, search terms, concerns, and interest in a redesign preview. If you follow a plan link, we also record your interest in the $150 or $300 SEO plan.
How We Use It
We use these details to evaluate your public website, prepare your report, and contact you about that request. Requesting an evaluation does not sign an SEO contract.
Storage and Contact
Requests are stored privately on PMALink's Plesk hosting server for the team to review. Automatic email notifications are not enabled. Public pages do not provide access to submitted requests. A hashed connection identifier is used to limit repeated submissions; the request record does not store your raw IP address.
Your Questions and Choices
For questions about a submitted request or to request its deletion, email info@pmalink.org. Please provide the request reference if you have it. Share business website and contact details rather than patient records.
What This Form Does Not Do
It does not request payment information, account passwords, or permission to start paid SEO. An evaluation request is not an automatically completed report. The team reviews the site and sends the report separately.
